Insurance and Sliding Scale for Behavioral Health

At Mazzoni Center, we are proud to integrate behavioral health support into our primary care services. Our Behavioral Health team offers brief consultations through Behavioral Health Consultants located in the medical center, as well as individual and group psychotherapy through our outpatient behavioral health program. We are committed to providing care that is affirmative, sex-positive, culturally humble, grounded in harm reduction principles, and focused on promoting patient self-determination. Eligibility for Behavioral Health Services

All patients have access to behavioral health consultations; however, outpatient behavioral health services are limited to patients engaged in primary care at Mazzoni Center who are uninsured or covered by Medicaid, Medicare, or Ryan White programs. At this time, we do not accept private or commercial insurance for outpatient behavioral health services.

Behavioral Health Services Offered

Behavioral Health Consultations include short-term, individualized behavioral health interventions focused on issues like stress, anxiety, and chronic health conditions, with referrals to external providers when appropriate.

Outpatient Behavioral Health services consist of long-term psychotherapy for eligible primary care patients, typically initiated through a referral made during a medical appointment by a Behavioral Health Consultant.

Psychiatry services include evaluations and medication management tailored to individual needs, with referrals made by a staff medical or mental health provider.

Sliding Fee Discount Program

We are committed to making care accessible to all patients, regardless of insurance or ability to pay. For those without adequate insurance, we offer a Sliding Fee Discount Program based on household size and income. Coverage through the program lasts up to 12 months and must be renewed annually, or sooner if there is a change in household income or size. Proof of gross annual income is required for all immediate family members. 

Acceptable forms of documentation include recent federal tax returns, W-2 or 1099 forms, or two recent pay stubs. Applications can be completed during registration or check-in through the patient portal.
