Mazzoni Center's medical services are in network with most Medicare and Philadelphia area Medicaid insurance plans, as well as many major insurances. We are NOT contracted to accept Qual Care, Cigna Connect EPO, Oscar, any out of state Medicaid, or Military/Armed Forces insurance plans such as Tricare and Champus. If you have an HMO insurance plan, Mazzoni Center or one of our providers must be listed with your insurance as your Primary Care Provider, or they will not cover the cost of your visit

Sliding Fee Discount Program
Mazzoni Center accepts all patients, regardless of their insurance status or ability to pay. We offer access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program for most services, with discounts available based on your current household size and income. The program can cover you for up to 12 months—a new application is required yearly or when your household size and income changes, whichever is sooner.
How do I qualify?
To qualify for the Sliding Fee Discount, you MUST show proof of gross annual income for all immediate family members living in your household. Gross income is ALL income from ALL sources before taxes. Applicants should provide a copy of the accepted income verification materials listed below.
Acceptable Income Verification
- Recent Federal Tax Return
- IRS Form W-2 or 1099
- Two (2) Current Pay Stubs
How Do I Get Started?
To begin the Sliding Fee Discount application process, simply complete the form during the registration/check-in process on the patient portal.
