Insurance and Sliding Scale for Behavioral Health

All primary care patients at Mazzoni Center have access to Behavioral Health services. Behavioral Health Consultations are available to every patient receiving medical care here, and we accept most major insurance plans for these visits. To confirm coverage, please check directly with your insurer or review their website before your appointment.
 
Outpatient behavioral health services, such as long-term psychotherapy, are more limited. These services are available only to patients engaged in Mazzoni Center’s primary care program who are uninsured or covered by Medicaid, Medicare, or Ryan White programs. At this time, we do not accept private or commercial insurance for outpatient psychotherapy.
 
Psychiatry services are also limited and available by referral from a medical or mental health provider when medication may be helpful. Care is provided by our psychiatrist in collaboration with our primary care team.
 

Sliding Fee Discount Program

At Mazzoni Center, we are committed to ensuring that quality health care is accessible to everyone.  For those without insurance, we offer a Sliding Fee Discount Program, which provides discounts based on household size and income.

The program covers eligible patients for up to 12 months. A new application is required annually or whenever there is a change in household size or income.

How do I qualify?

To qualify for the Sliding Fee Discount, you must show proof of gross annual income for all immediate family members living in your household. Gross income is all income from all sources before taxes. Applicants should provide a copy of the accepted income verification materials listed below.

Acceptable Income Verification

  • Recent Federal Tax Return
  • IRS Form W-2 or 1099
  • Two (2) Current Pay Stubs

How Do I Get Started?

To begin the Sliding Fee Discount application process, simply complete the form during the registration/check-in process on the patient portal.
