Sarah Ralston Foundation Funds Mazzoni Center’s Remote Patient Monitoring Program for a Second Year

We are excited to announce that the Sarah Ralston Foundation, which has a mission to “support and enhance the quality of life of vulnerable and underserved older adults residing in Philadelphia County,” has generous awarded a second year of Innovative Project Support Grant funding to continue the Remote Patient Monitoring Program launched by Mazzoni Center with the support of the Foundation in the spring of 2024.
Sarah Ralston Foundation’s Innovative Project Support Grants provide funding for a diverse group of nonprofit organizations committed to addressing the most pressing needs of vulnerable, underserved older adults across Philadelphia. These grants span a wide range of sectors, including healthcare, housing, digital literacy, and community engagement, with a focus on empowering this population and fostering resilience.
Launched this past April, Mazzoni Center’s “Remote Patient Monitoring” program is a game-changing approach to addressing the needs of the LGBTQ+ seniors in our health center practice who are facing chronic conditions, as eligible and appropriate patients can be equipped with items such as scales, blood pressure cuffs, and glucometers that, connected through the internet, enabling Mazzoni Center medicals providers the ability for ongoing monitoring, lessening the need for in-person visits.
“Remote patient monitoring provides real-time transmission of health data, like blood pressures, weights, and blood glucose levels, and notifies staff when they are outside set limits,” says Stefan Lynch, Mazzoni Center’s Director of Nursing and Integrated Services.“This supports data accuracy and supports our providers’ ability to make quick adjustments to care plans for eligible patients with chronic conditions like hypertension, congestive heart failure, and diabetes. A patient enrolled in our RPM program is able to collect health data in the comfort of their home, reducing the burden of frequent appointment scheduling and time spent traveling to and from our office.”
