Receiving an HIV diagnosis is a life changing experience. We know that getting a person living with HIV connected to medical care as soon as possible is the best way to ensure their long-term health and well-being.
Our Linkage to Care program is designed to support clients with an HIV diagnosis through every step of this process, to ensure they have all the information they need and can access the resources and support that are available. Regardless of your income or insurance status, we can help you access the individual care and support services that are most helpful for you, whether you are interested in services at Mazzoni Center or other outside providers.
Our Linkage Coordinator can assist you with connection to:
- HIV Health Care
- Case Management
- Partner Services
- STI screening
- Sexual health information
- Access to free condoms and lube
- Linkage to gender affirming services
- Behavior health services
- Primary care services
- Linkage to health insurance
- Linkage to employment resources
- Support with education goals
- Support with GED
- Housing and shelter resources
- Linkage to food bank services
- Support with transportation needs
Even if you are recently diagnosed and did not get tested by Mazzoni Center but would like to speak with us about your needs and options for HIV care and support, we can speak with you. If you have fallen out of medical care and would like to get reconnected, our staff can work with you and help get you back into care.
The Linkage to Care Program is a short-term program that lasts about 3 months.
